First, mount your photo on card stock. You can choose whichever color you want; however, white is usually the standard. Depending on where your child is, the information will be placed in a different location so you do not cover the actual image. Cut out a piece of card stock (can be a different color) in any shape you want, as long as it is large enough to write information in. Fill in the information, glue the shape on and you are done. If you want, you can add even more decoration including transfers, stamps, stickers and other items.

An alternating idea would be to connect two piece of cardstock with a ribbon on the bind and then place the picture on the front, a saying on the left side and then the information on the right side.
